FAITH, ACTIVITY, AND NUTRITION (FAN) PROGRAM MATERIALS

Faith, Activity, and Nutrition (FAN) is a 12-month program designed to help churches create lasting healthier environments for physical activity and healthy eating. The FAN program was developed in partnership with the 7th Episcopal District of the African Methodist Episcopal (AME) Church (i.e., South Carolina) with grant funding from the National Institutes of Health. With funding from the Centers for Disease Control and Prevention, FAN program materials were revised and used in a Dissemination and Implementation Study in partnership with the Fairfield Community Coordinating Council and the South Carolina Conference of the United Methodist Church. Most recently, the training for FAN was converted to an online format, and churches across the U.S. were enrolled in an implementation study of FAN using this online training.

The FAN program materials listed below are provided for use in your church or place of worship:

  • FAN Assessment and Planning Guide – This guide outlines the FAN program in detail. We recommend starting here if you wish to put FAN into place in your church or place of worship. Both an English and Spanish version of the guide are available
  • FAN Program Plan – Churches and other places of worship can use this document to plan the activities they will put in place throughout the upcoming year.
  • Planning Documents – These documents help churches and places of worship plan how to “launch” FAN in their church.
  • Monthly Materials – For each of the 12 months, you will find a packet of materials that we suggest using for that month. Included in the monthly materials is a newsletter (“FAN Monthly”), bulletin insert, and educational materials (e.g., handouts and posters) that you can share with your congregation. Many of the materials we include are freely available from credible health organizations. Some materials were developed as part of the FAN program. Where available, the packet also includes Spanish language versions of the materials.
