Application Form and Instructions

Thank you for your interest in the 2008 Physical Activity and Public Health Courses!  Please submit FOUR COPIES of the completed application form and required items to:

*The deadline for applications is May 16, 2008.

Please note that in Section B of the application you will be requested to submit a sponsor letter, a one or two page statement indicating basis of interest in the course, and a curriculum vitae (a resume is acceptable for the Practitioner Course).  FOUR COPIES of these items should be submitted along with the application form.  Please be sure to sign and date Section C of the application form.

Additional information regarding travel, lodging, and course fee payment will be mailed to you upon your acceptance into the course.
